Understanding Calf Strain Severity

In the acute phase (first 3-5 days), focus on maintaining ankle range of motion through gentle plantarflexion and dorsiflexion exercises. Pain should not exceed 3/10 during these movements. By day 5-7, isometric calf raises can begin—holding bodyweight on the affected leg for 30-45 seconds without moving through range.

Progressive loading continues with eccentric strengthening around week 2-3, using controlled lowering movements on a step. This phase is critical for restoring the calf’s ability to absorb force during the landing phase of running. Return to Running Criteria

Returning to running too early is the most common mistake runners make with calf strains. Before resuming running, you should be able to complete 25 single-leg calf raises on the affected side, hop without pain, and perform running-specific movements like high knees and butt kicks with symmetry.

Initial running should begin with short intervals at easy pace, potentially using an anti-gravity treadmill to reduce load while maintaining running-specific movement patterns. Progress gradually from 80% bodyweight to full bodyweight over 2-3 weeks while monitoring symptoms.

Preventing Recurrence

Calf strains often recur because the underlying causes are not addressed. Common contributing factors include inadequate warm-up, sudden increases in training load, insufficient calf strength relative to training demands, and altered running biomechanics that overload the posterior chain.

A structured strength programme focusing on calf endurance and eccentric capacity should continue beyond return to running. Runners training for longer distances should be able to complete 30+ single-leg calf raises as a baseline strength standard. Regular monitoring of training load progression—keeping weekly increases below 10%—also reduces recurrence risk.
